Getting the diagnosis right
Testosterone follows a daily rhythm and peaks in the morning. An afternoon draw can read low in a man whose levels are fine.
A proper workup includes two separate morning total testosterone measurements, plus free testosterone, LH and FSH, estradiol, a complete blood count, PSA when age-appropriate, and thyroid and metabolic markers.
That panel also answers a second question: whether the issue starts in the testes or in the signaling from the brain. The answer changes the treatment.
The main options
Injections
Weekly or twice-weekly injections are the most common approach. They are inexpensive, dependable, and easy to adjust.

Applied daily, these deliver steady levels and are simple to stop if needed.
The tradeoffs are absorption that varies between individuals and the need to avoid skin-to-skin transfer to a partner or child until dry.

Placed under the skin during a brief office visit, pellets release testosterone steadily for several months with no weekly routine.

Options that support your own production
For younger men or those who want to preserve fertility, medications that stimulate the body's own testosterone production may be a better fit than replacement.
Standard testosterone therapy suppresses sperm production in most men. That is reversible for many, but not guaranteed, so fertility plans belong in the first conversation rather than a later one.
What monitoring looks like
Labs are typically rechecked around six to eight weeks after starting, then a few times in the first year, then periodically.

A common mistake
Treating the number instead of the man.
A man can land at the top of the reference range and still feel flat if estradiol is off, sleep apnea is untreated, or thyroid function is low. Chasing a higher total testosterone in that situation rarely helps.
What to handle alongside treatment
Untreated sleep apnea suppresses testosterone and gets worse with therapy in some men. Excess weight increases conversion to estrogen. Heavy alcohol use, chronic stress, and poor sleep all pull in the wrong direction.
